Ordering Details
There are several ways to order from Mind Garden:
- Online - Order online simply by clicking on the appropriate icon--for either pdf
or paper 
format--
next to every product. The icons indicate the various formats in which you can buy an instrument. When you are ready to checkout, click on "checkout" on
your shopping cart page, and our secure commerce site
will process your order with your credit card (MasterCard or Visa only). We also take PayPal.
- Phone - Call us at (650) 322-6300. Please have a valid credit card (MasterCard, Visa, American Express, Discover) or a Purchase Order number ready when you call (We'll need you to fax or email us a copy of the P.O.) If you reach our voicemail, leave your name and number and tell us the best time to reach you.
- Order Forms by fax or mail. Printable copies of the Mind Garden order form are available here in pdf or MSDoc format (you can type into the MSWord version, save and email it to us as an attachment).
- If ordering by Purchase Order, please include a copy of the purchase order along with the Mind Garden Order form .
- If your order includes a PDF or a product with Online Survey, we need the name of the person who will own the PDF and the email of the person who will need to access our system for retrieval or survey set-up.
- If sending a check, your product will be shipped upon receipt of your check.

Payment Options
Mind Garden accepts payment by MasterCard or Visa, or PayPal via our website shopping cart. In order to use American Express or Discover cards, you must use an order form or call us (no Amex or Discover via the shopping cart). We also accept Purchase Orders and Wire Transfers (contact us for details on wire transfers), accompanied by a completed Mind Garden order form. Payments must be in U.S. dollars and checks must be drawn on funds from a U.S. bank.
Save time and avoid shipping charges
Regardless of whether you order the paper version or the PDF (electronic) version of an instrument, you will receive one copy of the instrument or document with license to reproduce the purchased quantity. However, by ordering the PDF/electronic version you will save time and avoid taxes and shipping charges.
PDF Delivery Time. Shortly after you order the PDF version and complete payment on your order, you will receive two emails from Mind Garden. The first email will be your sales receipt or invoice and the second email will contain instructions on how to retrieve your PDF (for multiple PDFs it is necessary to log in only once). Assuming there are no technical problems on your end or ours, PDFs will be available to you same day (not instantly!).
If by some chance you do not receive the instructional email, first check your Spam and Junkmail folders for an email from Mind Garden. If the email is not there, go to our home page and at the top, click on Transform. In the green box enter your "ship to" email address twice and create a password. Then you can view, download, and print the PDF(s) you purchased. You are always welcome to email us with questions by clicking here.
Storage and retrieval of PDFs. Your PDF will remain on our server for approximately one year.
PDF Files / "No Returns Policy"
Most of our instruments are available for purchase in 
PDF
format. PDF files can be read and printed on your computer with Acrobat reader (free), available
at Adobe. When a product is available in PDF format, you will see the
Acrobat icon
to the right of the product
title. It is important to know that this is exactly the document you want
because you cannot "return" a PDF document.
We'll say that again: Please be sure you've chosen the right document, because PDF purchases cannot be returned. If you have any questions about the preview procedure, please call or email Mind Garden.
Returns and Exchanges
- Reproduction licenses and PDF files may not be returned.
- Qualified returns will be accepted within thirty days of purchase.
- Returns must be sent to Mind Garden by certified mail or other traceable method.
- To receive credit, products must be in re-saleable condition and accompanied by a copy of the original invoice.
- Shipping charges are non-refundable.
Statistical Support
Mind Garden recommends that you purchase a manual for the instrument with which you are conducting research. The manual will assist in administration, scoring, interpretation, and normative comparisons, all of which are necessary for appropriate use of the instrument. Should you need additional statistical support, one source we highly recommend is Statistics Solutions, Inc. Statistics Solutions consists of a team of Ph.D. research methodologists and statisticians specializing in psychological survey instruments, research design, data analysis, sample size, conducting the appropriate statistics and formulating the results. You may contact them at any step of the process, however of course the earlier, the better. Click here to contact Statistics Solutions Inc. to learn how they can assist with your graduate or professional research. A good information resource is the NIH Behavioral and Social Science Research Methods Website. The website provides a centralized source of behavioral and social science research methods.
Thesis or Dissertation Appendices
With the purchase of reproduction licenses, you will receive a "Sample item" letter. This letter allows you to re-print up to the specified number of items of your choice in the appendix of your thesis or dissertation (the actual number allowed varies from 3-5 items depending on the instrument). This "Sample item" letter will be the last page of your reproduction license document. PLEASE NOTE that it is strictly against our policy to allow an entire instrument to be reproduced in a thesis or dissertation. Limitation of Liability
Mind Garden, Inc. will not be responsible or liable for the use or misuse of the materials or services offered in our catalog or website. The purchaser/user assumes all responsibility for use or misuse of the same. Mind Garden, Inc. will not be liable for any failure or delay in performance due in whole or in part to any cause beyond its reasonable control.
Mind Garden, Inc. makes no other warranties, express or implied, including warranties of merchantability or fitness for a particular purpose. In no event will Mind Garden, Inc., nor anyone else who has been involved in the creation or production of any of the printed works or software listed in this catalog, website or otherwise sold by Mind Garden, Inc., be liable for any indirect, special, or consequential damages arising out of the purchase or use of the materials or services from this catalog or website, notwithstanding any other provision or agreement between customer and Mind Garden, Inc. The customer's sole and exclusive remedy shall be correction or replacement of the printed work or of the software.
Putting an instrument on a non-Mind Garden survey website
Mind Garden provides Transform Online Survey for a number of our instruments and in fact multi-rater data collection is available for the MLQ. This allows you to avoid re-typing, reformatting and having to learn how to design surveys. Mind Garden can add your other instruments to our system and we can even customize your survey with demographic questions and special text and instructions (on a fee basis).
However we understand that sometimes it is necessary to put an instrument on an outside website for data collection. Please note that NOT ALL of our
instruments may be put on the web (The drop-down menu
on our online agreement form contains a complete list of instruments that may be put online). PLEASE NOTE THAT DISTRIBUTING AN ENTIRE INSTRUMENT IN EITHER THE TEXT OF AN EMAIL OR AS AN EMAIL ATTACHMENT IS STRICTLY PROHIBITED.
If you would like to request permission to put one of our instruments on an outside website (for data collection only, no reports), we require the completion of the Online Use application (See link below). A minimum purchase of 50 reproduction licenses is required prior to completing the form, and you'll need your order number as a reference.
- The name of the psychological instrument
- The name of your research project
- Your name and institution
- A statement that you have compensated Mind Garden for at least 50 reproductions and will compensate Mind Garden for every administration of the online form
- A statement that you will put the instrument copyright on every page containing question items from this instrument
- A statement that you will remove this form from the internet/survey site at the conclusion of your data collection
- This form must not be on the "open web". You must require a unique login and password for every respondent (Survey Monkey calls this the Email Invitation Collector). State how you are putting the form up that requires a login. It is important to specify the data-collection service that you are using (e.g., university intranet tool, Survey Monkey, PsychData, etc.). IMPORTANT NOTE:You cannot send Mind Garden instruments in the text of an email or as a PDF file to participants.
