Manual License to Use

This kit was designed to meet the needs and expectations of managed mental health care (MMHC) and health care reform for mental health providers. It allows for more documented concrete data of treatment outcome. The kit includes: Quality of Emotional Life Self-Report (QELSR) (Questionnaire), clinical summary forms, a progress note form, and data sheets used to provide a psychotherapy outcome profile.

There are two versions: Adult Form (ages 16 and older) and Child Form (ages 15 and younger).

You need a Manual if...

... you want to view the instrument prior to purchasing a License to Reproduce/Administer or Online Survey.

... you need details on Reliability, Validity, Administration, Scoring and Interpretation.

... you need to submit a non-reproducible copy of the instrument to your IRB or advisor for approval.

"The Psychotherapy Outcome Kit includes a psychotherapy outcome scale called Quality of Emotional Life Self-Report (QELSR), clinical summary forms, a progress note form, and data sheets used to provide a psychotherapy outcome profile.

The clinical report forms include a QELSR summary section of the patient's individual QELSR item scores and the QELSR total score. The clinical summary forms can be filled out when treatment begins, at termination of treatment, and at other times as needed. They can be used for ongoing record keeping, sending out to referral sources, and, as stated above, for added documentation for MMHC certification requests.

The psychotherapy outcome profile can be used for quality assurance within a private solo or group practice or agency setting. In addition, it can be included in application packets for MMHC preferred provider panels or for applications to health maintenance organizations and for ongoing marketing of services."
