The Psychotherapy Outcome Kit (POK) kit was designed to meet the needs and expectations of managed mental health care (MMHC) and health care reform for mental health providers. It allows for more documented concrete data of treatment outcome. The kit includes: Quality of Emotional Life Self-Report (QELSR) (Questionnaire), clinical summary forms, a progress note form, and data sheets used to provide a psychotherapy outcome profile.
